The Guiding Pulse website is here to support people dealing with hypertension to take charge of their health. It offers reliable information and practical tools for making lifestyle changes, like healthier eating and more exercise, along with improving communication with healthcare providers. By gathering input from patients and experts, the site provides educational content and interactive features to help users manage their condition effectively. Our goal is to make trustworthy health advice accessible to everyone who needs it.

Introduction

High blood pressure, also known as hypertension, is a major risk factor for cardiovascular diseases (CVD) and affects a significant portion of the population in North America. Lifestyle changes are highly recommended for managing hypertension, as uncontrolled blood pressure can lead to serious health issues such as heart attacks and strokes. Unfortunately, people with lower socioeconomic status or without insurance often struggle to manage their hypertension compared to those with better resources.

Project Rationale

The Guiding Pulse project is part of a larger initiative called C-CHANGE, sponsored by Sunnybrook Hospital since 2017. C-CHANGE aims to provide comprehensive guidelines for primary care practitioners dealing with patients who have multiple health conditions. Our project focuses on two main areas: clinical information and patient education. By taking a patient-centered approach and considering various systemic factors, we aim to address the challenges individuals face in managing their health effectively.
